A referral for home health services is sent to our intake department via call, scan, fax, or written order from your MD.

Step 2

Our intake department verifies your insurance for eligibility.

Step 3

Our schedulers staff the needed clinicians and contact your physical therapist to confirm your information and obtain any additional information or requests.
